TUMOR  LYSIS  SYNDROME

 

Etiology:

  1. Neoplasm (some pts are unaware of  having CA)

S & S:

  1. Non specific or may be related to underlying disease  or  to complications of lysis such as HyperK, HypoCalcemia

DX:

  1. SMA reveals HyperK, HyperUricemia, HypoCalcemia
     
  2. EKG may have peaked T waves related to HyperK and prolonged QT secondary to HypoCalcemia
     

Treatment:

  1. Aggressive hydration
     
  2. Alkalinize urine to pH is 7.0 if hyperuricemia is present. If one uses bicarbonate to achieve this goal, careful monitoring of Ca is advised.
