ITP

 

Overview:

  1. Circulating IgG antiplatelet antibodies

Classification:

  1. Childhood variant: 1-2 wks. s/p viral infection
     
  2. Adult  variant:  Circulatory AB are present with consequent thrombocytopenia

S & S:

  1. Bleeding tendencies such as epistaxis, bruising, ICH, menorrhagia, G(+) stools
     
  2. Patechiae, purpura. No  splenomegaly.

DX:

  1. ¯ in plt count (platelets). The other line of  blood cells are not affected.
     
  2. Check fundi especially if pt c/o of visual disturbance.

Therapy:

  1. Prednisone 1-2mg/kg PO qd
     
  2. Gamma Immunoglobulin IV (400 mg/kg)
     
  3. Danazol 200mg TID PO
     
  4. Immunosoppressive therapy with Vincristine or Cyclophosphamide
     
  5. Splenectomy
     
  6. Plt transfusion is not beneficial since they are destroyed by AB. Yet, if pt is bleeding, plt transfusion is considered.
