DIC = DISSEMINATED  INTRAVASCULAR  COUGULOPATHY

 

Overview:

  1. Consumption of fibrin, platelets and coagulation factors
     
  2. Microangiopathic hemolytic anemia
     
  3. Severe underlying disease (malignancy, sepsis, pregnancy, trauma, etc.)

S & S:

  1. Bleeding with or without thrombotic component. Bleeding can present with hematuria, GI bleed, hemoptysis, patechiae. Thrombotic manifestations present with worsening renal failure, bowel, myocardial and pulmonary infarction, ischemic CVA.

DX:

  1. plt count
     
  2. > PT/PTT
     
  3. D-dimers, FDP
     
  4. Schistocytosis on peripheral smear

Treatment:

  1. IVF, O2
     
  2. FFP, platelet transfusion, cryoprecipitate (contains  fibrinogen and factor VIII)
     
  3. Treatment of underlying problem
     
  4. Heme-Onc and ICU consult
