DIC = DISSEMINATED INTRAVASCULAR COUGULOPATHY Overview:
- Consumption of fibrin, platelets and coagulation factors
- Microangiopathic hemolytic anemia
- Severe underlying disease (malignancy, sepsis, pregnancy, trauma, etc.)
S & S:
- Bleeding with or without thrombotic component. Bleeding can present with hematuria, GI bleed, hemoptysis, patechiae. Thrombotic manifestations present with worsening renal failure, bowel, myocardial and pulmonary
infarction, ischemic CVA.
DX:
- plt count
- > PT/PTT
- D-dimers, FDP
- Schistocytosis
on peripheral smear
Treatment:
- IVF, O2
- FFP, platelet transfusion, cryoprecipitate (contains fibrinogen and factor VIII)
- Treatment of underlying problem
- Heme-Onc and ICU consult
